Qualifications

• Hospice care experience of 1+ year is required.

Responsibilities

• Provide psychosocial assessments of patients and families.
• Develop bereavement plan of care for families/caregivers.
• Document assessments and MSW interventions in the EMR.
• Collaborate with IDT to plan social work care.
• Educate patients and families respecting beliefs and values.
• Support decisions and communications about end-of-life care.
