Qualifications

• Master's degree in Social Work required.
• LSW and/or LCSW licensure in IDFPR region required.
• Driver's License and auto insurance proof required.

Responsibilities

• Provide psychosocial assessments of patients and families.
• Develop bereavement plan of care for family needs.
• Educate patients and families respecting faith and culture traditions.
• Document assessments and interventions in EMR.
• Collaborate with IDT to implement social work plan of care.
• Support patient decisions and life care planning.
• Assist with transportation barriers and referrals for basic needs.
