Responsibilities

• Provide psychosocial assessments of patients and families to identify emotional, social, and environmental strengths and problems.
• Develop bereavement plan of care to address family caregiver needs.
• Interpret and communicate patient/family beliefs and culture; educate to overcome barriers.
• Document psychosocial assessments, MSW interventions and care plans in the EMR.
• Develop social work plan with IDT collaboration; supports care decisions and end-of-life planning.
