Job summary

Carle Health is seeking a Home Services Social Worker to identify psychosocial needs of patients and families and collaborate with interdisciplinary teams. The role includes hospice-related responsibilities, bereavement planning, and ED/EMR documentation, with emphasis on culturally sensitive care and family support.

Master's degree in Social Work is required, along with hospice experience of 1+ years. Illinois-licensed social workers (LSW/LCSW) and driving/insurance prerequisites are noted;

Qualifications

• Hospice experience of 1+ years is required.
• Licensure as LSW or LCSW is required where applicable.

Responsibilities

• Perform psychosocial assessments of patients and families.
• Develop and implement a bereavement plan of care.
• Communicate cultural and spiritual needs to the care team.
• Educate patients and families to overcome barriers and meet needs.
• Document assessments, financials, and MSW interventions in the EMR.
• Collaborate with IDT to plan social work care.
