Job summary

Carle Health is seeking a Home Services Social Worker to identify psychosocial needs of patients and families, coordinate resources and provide short-term counseling in a collaborative interdisciplinary team.

Role requires Hospice experience of 1+ years and offers a sign-on bonus. Licensure paths include LSW or LCSW per Illinois regulations, with a valid Driver's License and auto insurance proof. Hours vary with census and program needs.

Qualifications

• Hospice experience of 1+ years.
• MSW/LCSW licensure pathways as required by state (IDFPR).
• Driver's License and proof of Auto Insurance.

Responsibilities

• Provide psychosocial assessments of patients and families to identify emotional, social, and environmental needs.
• Develop a bereavement plan of care for family members/caregivers.
• Communicate patient/family faith and cultural traditions and adjust care accordingly.
• Educate patients and families to overcome barriers and meet learning needs.
• Document assessments and social work interventions in the EMR in a timely manner.
• Develop and implement a social work plan to optimize support, benefits, and coping, and adjust as needed.
