Job summary

Carle Health is seeking an Home Services Social Worker to identify psychosocial needs of patients and families through assessment and to support resource identification, counseling, and crisis intervention as part of an interdisciplinary team. Role requires hospice experience (1+ years) and offers a sign-on bonus.

Masters in Social Work with LSW/LCSW licensure per IDFPR and a valid driver’s license are required.

Qualifications

Master's degree in Social Work from an accredited program.
Licensed in Illinois (LSW or LCSW) per IDFPR requirements.
Driver's license and auto insurance proof required.
Hospice care experience is required (1+ years).

Responsibilities

Provide psychosocial assessments of patients and families.
Develop bereavement plan of care addressing family needs.
Educate patients and families, overcoming barriers and tailoring to learning needs.
Document assessments, MSW interventions, and care plans in EMR.
Collaborate with IDT and support end-of-life decisions.
Identify and address spiritual/cultural needs impacting care.
Assist with transportation barriers and benefit referrals.
Support applications for Medicaid and other programs; refer for home services.
