Job summary

Carle Health is seeking a Home Services Social Worker to assess and address the psychosocial needs of patients and families within a hospice-focused home care setting. This role collaborates with an interdisciplinary team to identify resources, provide counseling, and support outcomes aligned with patient goals.

The position requires a Master’s Degree in Social Work, licensure (LSW or LCSW) under IDFPR guidelines, and a valid driver’s license.

Qualifications

• MSW required with licensure as LSW or LCSW
• Hospice experience of 1+ years is required
• Driver's license and auto insurance required

Responsibilities

• Provide psychosocial assessments of patients and families to identify emotional, social, and environmental needs
• Develop and implement a bereavement plan of care
• Collaborate with IDT to ensure plan of care aligns with patient goals and cultural values
• Document assessments and interventions in the EMR in a timely manner
• Assist with transportation barriers, referrals for food/clothing, and applications for Medicaid/SSDI
