Responsibilities

• Provide psychosocial assessments to identify strengths and problems in diagnosis and life context.
• Develops and implements a bereavement plan of care for families and caregivers.
• Educates patients and families, respecting faith, culture, and learning style.
• Documents assessments and social work interventions in the EMR; coordinates with IDT.
• Supports decision-making, end-of-life planning, and patient self-determination.
