Job summary

Carle Health is seeking a Home Services Social Worker to identify psychosocial needs of patients and families through assessment and collaboration with interdisciplinary teams. Hospice experience of 1+ year is required. Licensure as LSW or LCSW and a valid driver's license are required.

The role includes developing bereavement plans, coordinating community resources, and documenting in the EMR. Hours vary with census and program needs, with an emphasis on compassionate end-of-life care.

Qualifications

• Master's Degree in Social Work required.
• Licensure as LSW or LCSW required.
• Valid driver's license and auto insurance may be required.

Responsibilities

• Provide psychosocial assessments of patients and families to identify emotional, social, and environmental needs.
• Develop bereavement plan of care for family members and caregivers.
• Document psychosocial assessments and MSW interventions in the EMR.
• Collaborate with IDT to develop and implement social work plan of care.
• Support patient decisions and end-of-life care planning.
