Job summary

Affinity Hospice Management LLC seeks an MSW to implement and oversee psychosocial care for hospice patients and families. The role works within the interdisciplinary group to assess needs, plan interventions, and monitor outcomes.

The MSW will provide crisis intervention, education on advance directives, and family support while coordinating with community agencies to ensure comprehensive, compassionate care.

Qualifications

• Excellent verbal and written communication skills.

• Accurate documentation and record-keeping.

Responsibilities

• Assesses the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment.
• Carries out social evaluations, including family dynamics and caregiver abilities, planning interventions.
• Assesses for cultural needs and educates the interdisciplinary group on appropriate approaches.
• Educates patients and families on advanced directives.
• Contributes to development of the plan of care with patient/family input.
• Assists physicians and team in understanding social and emotional factors related to health problems and death/dying.
• Assists with planning funeral arrangements, financial, legal, and health care decisions.
• Completes and maintains clinical notes; communicates patient needs to the team.
• Coordinates with physicians and community health professionals for care planning and referrals.
• Provides information to patients and families and serves as liaison with agencies.
• Maintains collaborative relationships with staff and external resources.
• Maintains and develops contracts with agencies as resources for patients and staff.
• Performs on-call duties as defined by policy.
• Supports RN Case Manager in arranging equipment and services.
• Engages in continuing education and professional development.
• Performs other duties as assigned.
