Job summary

Affinity Health Management is seeking a licensed MSW to implement standards of care for hospice medical social work services. Qualifications

• Master’s degree in Social Work (MSW) from an CSWE-accredited program.
• Strong written and verbal communication skills.
• Ability to organize and manage multiple tasks in a clinical setting.
• Experience in hospice care and psychosocial assessment is preferred.

Responsibilities

• Assess psychosocial status of patients and families in relation to terminal illness and communicate findings to the care team.
• Conduct social evaluations, address high-risk situations, and provide crisis intervention when needed.
• Educate interdisciplinary team about cultural needs and advanced directives.
• Participate in developing and updating individualized plans of care with patients and families.
• Document changes and outcomes in clinical notes; maintain accurate records.
• Coordinate care with physicians and community resources; provide information to patients and families.
• Serve as liaison between patients, families, and community agencies; maintain collaborative relationships.
• Develop and manage contracts with public and private agencies as needed.
• Participate in on-call duties and support RNs and other team members.
• Fulfils responsibilities for ongoing professional development and meeting quality improvement goals.
• Meet requirements for licensing, driving, and CPR certification as applicable.
