Job summary

Affinity Hospice Management LLC is seeking a MSW to implement and oversee psychosocial care for hospice patients and families. You will work within the interdisciplinary group, assess needs, plan interventions, and support advance directives and bereavement processes.

Qualifications

• Master's degree in Social Work (MSW) required.
• Minimum of one year experience in health care; hospice experience preferred.
• Excellent verbal and written communication skills.
• Ability to educate interdisciplinary teams and patients/families.
• Strong organizational and time-management abilities.
• CPR certification required.
• Must be a licensed driver with reliable transportation.
• Covid-19 vaccination status per policy.
• Understand hospice philosophy and issues related to death/dying.

Responsibilities

• Assess psychosocial status of patients and families in terminal illness and communicate findings to the care team.
• Conduct social evaluations including family dynamics and caregiver abilities; plan interventions.
• Educate on advanced directives and plan of care; attend interdisciplinary meetings.
• Support patient/family coping, provide crisis intervention when needed.
• Assist in coordinating care, planning funeral arrangements, and financial/legal responsibilities.
• Maintain accurate clinical notes and coordinate with physicians and community agencies.
• Participate in on-call duties and continuing education.
• Contribute to quality improvement initiatives.
• Ensure safe environment and continuity of care for patients.
