Job summary

Affinity Hospice Management LLC seeks an MSW to implement psychosocial care standards for hospice patients and families. The role is a core member of the interdisciplinary group, providing psychosocial support and planning within the established care plan.

The successful candidate will assess psychosocial needs, coordinate with the team, educate on cultural considerations and advance directives, and participate in care planning and on-call duties.

Qualifications

• MSW from a Council on Social Work Education-accredited program.
• Minimum of one year experience in health care; hospice experience preferred.
• Hospice care experience preferred.
• Demonstrates good verbal and written communication, and organization skills.
• Contingent upon satisfactory references and criminal background checks as required.
• Prolonged walking or standing; visual and hearing acuity needed for social work tasks.
• Must be a licensed driver with a valid insured vehicle.
• CPR certification is required (hands-on demonstration preferred).
• Covid-19 vaccination is required.

Responsibilities

• Assess psychosocial status of patients and families/caregivers related to terminal illness; communicate findings to the care team.
• Carry out social evaluations including family dynamics, caregiver abilities, and crisis interventions.
• Educate the interdisciplinary group on cultural needs and advance directives.
• Assist in developing individualized care plans; participate in interdisciplinary meetings.
• Help team understand social and emotional factors affecting health and death/dying.
• Assist with funeral planning, financial, legal, and health-care decisions.
• Complete and maintain accurate clinical notes; coordinate with physicians and community agencies.
• Participate in on-call duties and support case management for equipment and services.
• Pursue ongoing professional development and continuing education.
• Attend interdisciplinary group meetings and contribute to care continuity.
