Job summary

Affinity Hospice Management LLC is seeking a Medical Social Worker (MSW) to implement standards of care for hospice psychosocial services and support patients and families throughout the illness trajectory.

The MSW collaborates with the Interdisciplinary Group, conducts psychosocial assessments, educates on advanced directives, and assists with planning, crisis intervention, and end-of-life decision making.

Qualifications

• Master’s degree in Social Work (MSW) from a CSWE-accredited school.
• Demonstrates strong communication and organizational skills.
• CPR certification required and possession of a valid driver's license.

Responsibilities

• Assess psychosocial status of patients and families and communicate findings to the care team.
• Conduct social evaluations including family dynamics and coping needs; provide crisis intervention.
• Educate on advanced directives and participate in care planning with the Interdisciplinary Group.
• Assist with planning for funeral arrangements, financial, legal, and health care decisions.
