Qualifications

• Master's degree in Social Work (MSW) from an accredited program.
• 1+ year healthcare or hospice experience preferred; understands hospice philosophy and death/dying issues.
• Licensed driver with insured automobile meeting state requirements.
• CPR certification (in-person verification required).
• Must pass background checks and satisfy references as requested.

Responsibilities

• Assess psychosocial status of patients and families and communicate findings to the care team.
• Counsel patients and families; provide crisis intervention as needed.
• Educate on advanced directives and participate in the care plan development.
• Assist in understanding social and emotional factors related to health problems and death/dying.
• Participate in interdisciplinary group meetings and coordinate with team members.
