Job summary

Affinity Hospice Management LLC is seeking a Medical Social Worker (MSW) to implement care standards for hospice patients and families. You’ll join the interdisciplinary group to provide psychosocial support based on assessments and an individualized plan of care.

This role emphasizes professional judgment in monitoring psychosocial needs and coordinating services. Responsibilities include psychosocial assessment, family counseling, crisis intervention, education on directives, and collaboration

Qualifications

• MSW degree from a CSWE-accredited program.
• Minimum one year healthcare experience; hospice preferred.
• Strong verbal and written communication skills.
• Good organization and planning abilities.
• Valid driver’s license and reliable vehicle.
• CPR certification required; in-person demonstration preferred.
• COVID-19 vaccination is required.

Responsibilities

• Assess psychosocial status of patients and families and communicate findings to the care team.
• Conduct social evaluations, assess caregiver abilities and plan interventions.
• Educate the care team about cultural needs and advanced directives.
• Participate in the development of the plan of care with patient and family.
• Assist in understanding emotional factors and plan for funeral and legal matters.
• Document clinical notes and coordinate information with physicians and agencies.
• Attend interdisciplinary group meetings and provide continuity of care.
• Act as liaison with community resources and maintain contracts with agencies.
• Serve as required as on-call and support the RN Case Manager.
