Job summary

Affinity Hospice Management LLC is seeking a Medical Social Worker (MSW) to implement standards of care for hospice patients and families. You will join an Interdisciplinary Group to provide psychosocial support based on ongoing assessments and plan of care, ensuring compassionate, client-centered service.

The role involves psychosocial assessments, crisis intervention, education on culture and advanced directives, and collaboration with the care team to align with patient goals and comfort.

Qualifications

• Master's degree in Social Work (MSW) from a CSWE-accredited program.
• Minimum of one year experience in health care; hospice experience preferred.
• Licensed driver with an automobile that is insured.
• CPR certification.
• Covid-19 vaccination.

Responsibilities

• Assess psychosocial status of patients and families; communicate findings to the care team.
• Conduct social evaluations, plan interventions, and provide crisis intervention when needed.
• Educate on culture-related needs and advanced directives.
• Support development of the plan of care and participate in interdisciplinary meetings.
• Assist in planning funeral arrangements and financial/healthcare decisions.
• Maintain accurate clinical notes and coordinate information with physicians and community agencies.
• Provide continuity of care and support emotional needs of patients and families.
