Qualifications

• Requires MSW from CSWE-accredited program.

Responsibilities

• Assess psychosocial status of patients, families and caregivers and inform the care team.
• Provide crisis intervention and family counseling as needed.
• Educate on advanced directives and cultural considerations.
• Assist in development and update of the individualized plan of care.
• Collaborate with hospice team to address emotional and social needs.
