Job summary

Affinity Hospice Management LLC is seeking a Medical Social Worker (MSW) to implement standards of care for hospice patients and families. You’ll work as part of the interdisciplinary group, providing psychosocial support based on ongoing assessments and planned interventions.

The role involves conducting psychosocial evaluations, counseling, and coordinating care, including advanced directives and funeral planning, within a hospice setting.

Qualifications

• A graduate of a Master’s program in Social Work (MSW) from a school accredited by the Council on Social Work Education
• Minimum of one (1) year experience in health care, hospice experience preferred. Understands hospice philosophy, and issues of death/dying.
• Experience in hospice care preferred.
• Demonstrates good verbal and written communication, and organization skills.
• Once an offer of employment is made, it is contingent upon satisfactory references, as requested, and criminal background checks by regulation.
• Prolonged or considerable walking or standing. Visual acuity and hearing to perform required social work skills.
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.

Responsibilities

• Assesses the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment and communicates findings to the nursing and care team.
• Carries out social evaluations, including family dynamics, caregiver abilities, communication patterns, and plans intervention when risks are identified.
• Assesses for cultural needs and educates the interdisciplinary group on relevant traditions and taboos.
• Educates patients and families on the preparation of advanced directives.
• Participates in developing the individualized plan of care and attends interdisciplinary group meetings.
• Assists the physician and team in understanding social and emotional factors related to health problems and death/dying.
• Assists family and patient in planning for funeral arrangements, finances, legal, and health-care decisions.
• Maintains accurate clinical notes and records changes or outcomes as patient conditions evolve.
• Communicates with physicians and community health professionals to coordinate the care plan.
• Participates in on-call duties and supports care coordination as required.
