• High School Diploma or equivalent is required.
• Licensed Practical Nurse (LPN) license and valid clinical license.

Responsibilities

• Schedule patients to field clinicians and coordinate care with patients, physicians, referral sources, caregivers, and staff to ensure continuity of care.
• Manage the on-call notebook and hospitalization logs to enhance communication among stakeholders.
