Job summary

Andwell Health Partners is seeking an RN to provide skilled nursing in patients’ homes within Farmington and surrounding towns. You’ll work under a physician’s plan of care, develop measurable care outcomes with patients and families, and educate patients on rights and decision making in their care plan.

The role requires an active Maine RN license, CPR certification, and at least one year of clinical nursing experience. A reliable car with insurance is also needed.

Qualifications

• Current R.N. license in the State of Maine.
• ANA certification in Home Health or Community Health Nursing encouraged.
• One year of recent clinical nursing experience required.
• Current CPR card required.
• A reliable car with insurance.

Responsibilities

• Provide skilled nursing services to the patient in their own home under the direction of a physician’s plan of care.
• Develop plans of care with realistic/measurable outcomes in collaboration with the patient/family.
• Provide education and promote patient’s rights and responsibilities for informed decision making in the plan of care.
• Assess and identify the patient’s holistic health care needs.
• Demonstrate competency in assessment and technical skills.
• Possess and utilize knowledge of pain control & symptom management.
• Work 30 hours per week and get paid for 36 hours per week!
