Job summary

Andwell Health Partners is seeking an RN for hospice care in the Farmington, ME area. The role is weekend shift, delivering skilled nursing services in patients' homes and community facilities, with 36 paid hours per week in a full-time schedule.

Responsibilities include planning and implementing care plans, educating patients and families, assessing needs, and managing pain and symptoms under a physician's orders. Must have Maine RN license and CPR; driving required.

Qualifications

• Current R.N. license in the State of Maine.
• ANA certification in Home Health or Community Health Nursing encouraged.
• Current CPR card required.
• One year of recent clinical nursing experience required.
• A reliable car with insurance.

• Provide skilled nursing services to the patient in their own home under the direction of a physician’s plan of care.
• Develop plans of care with realistic/measurable outcomes in collaboration with the patient/family.
• Provide education and promote patient’s rights and responsibilities for informed decision making in the plan of care.
• Assess and identify the patient’s holistic health care needs.
• Demonstrate competency in assessment and technical skills.
• Possess and utilize knowledge of pain control & symptom management.
• Work 30 hours per week and get paid for 36 hours per week!
