Job summary

Andwell Health Partners in Maine is seeking an experienced Registered Nurse to provide skilled nursing services in patients' homes, following a physician's plan of care. The role offers 32 hours per week with a 3:00pm-11:30pm shift, enabling meaningful, year-round community care and flexible scheduling within a supportive home-health setting.

You will develop care plans, educate families, assess needs, and apply pain control and symptom management knowledge.

Qualifications

• Current RN license in the State of Maine.
• ANA certification in Home Health or Community Health Nursing encouraged.
• Current CPR card required.
• One year of recent clinical nursing experience preferred.

Responsibilities

• Provide skilled nursing services to the patient in their own home under the direction of a physician’s plan of care.
• Develop plans of care with realistic/measurable outcomes in collaboration with the patient/family.
• Provide education and promote patient’s rights and responsibilities for informed decision-making in the plan of care.
• Assess and identify the patient’s holistic health care needs. Demonstrate competency in assessment and technical skills.
