Job summary

Andwell Health Partners is seeking an experienced Hospice Admission RN to deliver skilled nursing care in patients' homes within Lewiston and surrounding towns.

You will work under a physician's plan of care, developing care plans and educating families to support informed decisions.

Qualifications

• RN license in Maine required.
• ANA certification in Home Health or Community Health Nursing encouraged.
• Current CPR card required.

Responsibilities

• Provide skilled nursing services to patients in their homes under physician plan of care.
• Develop plans of care with measurable outcomes in collaboration with patient/family.
• Provide education and promote patient rights and responsibilities in the plan of care.
• Assess and identify patient holistic health care needs.
• Demonstrate competency in assessment and technical skills.
• Utilize knowledge of pain control and symptom management.
