Job summary

Four Seasons Hospice seeks an RN – Clinical Team Leader to guide an interdisciplinary hospice team in the West region, overseeing patient care coordination and quality initiatives. The role reports to the Regional Director and involves leadership of CNAs and RNs in a dynamic post-acute setting.

The position requires an NC RN license, 3+ years of clinical experience, and a track record of collaboration and caregiving excellence.

Qualifications

• Bachelor of Science in nursing or healthcare related field preferred.
• Three years relevant clinical experience.
• Two years of clinical leadership experience (preferred).
• Demonstrated skill/expertise in pain and symptom management.
• Demonstrated ability to effectively collaborate and facilitate the work of an interdisciplinary group.
• Understanding of reimbursement, regulatory requirements, and Medicare Conditions of Participation.

Responsibilities

• Provide clinical, administrative, and economic leadership for the interdisciplinary hospice team.
• Guide the IDG to address physical, psychosocial, environmental, spiritual, safety and developmental needs.
• Participate in quality improvement activities and promote a culture of improvement.
• Develop and update individualized plans of care in collaboration with the IDG.
• Ensure comprehensive initial assessment and timely admission to hospice care.
