Job summary

Four Seasons RN – Clinical Team Leader oversees hospice/palliative care services, and leads the Interdisciplinary Group to ensure coordinated care and high-quality outcomes for patients and families.

The role requires a BSN, 3 years of clinical nursing experience, and 2 years in leadership/management with strong teamwork and communication skills; schedule is primarily weekday with some weekend coverage as needed.

Qualifications

• BSN required; active RN license.
• Minimum 3 years of clinical nursing experience.
• 2 years of clinical leadership/management experience preferred.
• Case management experience in home-care, hospice, or palliative care helpful.

Responsibilities

• Provide clinical leadership for the interdisciplinary hospice team.
• Coordinate patient care plans with the IDG and ensure regulatory compliance.
• Lead quality improvement activities and promote evidence-based practice.
