Job summary

Four Seasons is seeking an RN Case Manager to coordinate nursing plans of care, supervise LPNs and aides, and manage a caseload of about 15 patients with varying needs. The role emphasizes adherence to regulations, HIPAA, and effective communication with healthcare teams.

The position requires clinical experience and a valid RN license, with full-time, part-time, and PRN options. Expect a 40-hour week with occasional on-call responsibilities in a supportive team environment.

Qualifications

• Two years of clinical experience preferred.
• Knowledge of Federal, State and Local regulations and Medicare Conditions of Participation.
• HIPAA privacy rules and regulations.
• Knowledge of electronic medical records systems.
• Excellent written and oral communication skills.
• Ability to work under pressure to meet deadlines.
• Ability to take initiative and utilize innovative techniques.
• Ability to work flexible hours and coordinate multiple projects.

Responsibilities

• Coordinates the nursing plan of care and provides direct nursing care.
• Supervises LPN and Hospice Aide staff in delivering nursing care.
• Manages an assigned caseload of approximately 15 patients with varying acuity.
• Assess the patient's health status and treatment responses.
• Record and report nursing assessments and plans of care.
• Educate patients and families about care plans and treatments.
• Coordinate with healthcare providers to determine patient-centered plans.
• Implement prescribed regimens and ensure safety in care delivery.
• Supervise and train staff on nursing procedures and programs.
• Maintain safe and effective nursing care practices.
