Job summary

The Care Team Hospice is seeking an experienced RN to step into Clinical Team Leader, partnering with the Branch Director to guide daily clinical operations and staffing. You will coach nurses and interdisciplinary team members, review care plans and documentation, and promote continuity and regulatory compliance across the branch.

Qualifications

• Active and unrestricted RN license in the state of practice.
• Graduate of an accredited nursing program; ADN or BSN acceptable.
• Minimum of two years RN experience within the last 36 months.
• Hospice or home health experience strongly preferred.
• Previous supervisory or clinical leadership experience preferred.
• Strong clinical judgment and coaching ability.
• Excellent communication and organizational skills.

Responsibilities

• Partner with the Branch Director to oversee daily clinical operations and staffing.
• Provide leadership, coaching, and clinical guidance to nurses and interdisciplinary team.
• Review care plans, documentation, admissions, discharges, and workflows for quality and compliance.
• Monitor quality via audits and metrics; drive improvements.
• Support onboarding, competency development, and ongoing education.
• Provide direct patient care and on-call support as needed to ensure continuity.
