Four Seasons in Western North Carolina seeks an RN – Clinical Team Leader to provide nursing leadership for CNAs and RNs in the West region, overseeing hospice and palliative care delivery and coordinating the Interdisciplinary Group.

You will lead the IDG, drive quality improvement, ensure compliant care, and mentor staff while aligning with Four Seasons values; the role requires RN licensure in NC, BSN preferred, and hospice care experience.

Formación

• Ability to lead interdisciplinary hospice team and coordinate care.
• Knowledge of HIPAA, regulations, and Medicare Conditions of Participation.
• Strong organizational and interpersonal skills.
• Experience in hospice or palliative care settings.

Responsabilidades

• Provide clinical, administrative, and economic leadership for the hospice team.
• Ensure the IDG develops and implements patient- and family-centered care plans.
• Monitor quality via audits, PIPs, and continuous improvement activities.
• Coordinate care across multiple sites within the West region.
• Ensure regulatory compliance and accurate documentation.
• Mentor staff and promote professional development.
