Four Seasons is seeking an RN – Clinical Team Leader to provide nursing leadership for the hospice IDG in the West region. You will oversee care coordination, adherence to standards, and collaboration with Partner Relations Managers to support mission and quality goals.

The role requires leadership experience, clinical expertise in pain and symptom management, and a commitment to outpatient and in-home hospice care within a team-driven environment.

Competenze

• Bachelor of Science in Nursing or healthcare-related field preferred.
• Experience leading an interdisciplinary hospice team is preferred.
• Knowledge of Medicare Conditions of Participation and regulatory requirements.

Mansioni

• Provide clinical, administrative, and economic leadership for the interdisciplinary hospice team.
• Coordinate care plans and ensure high-quality hospice services for patients and families.
• Participate in continuous quality improvement activities and regulatory compliance.
• Lead IDG/IDT meetings and collaborate across departments to meet patient goals.
• Mentor staff, supervise assignments, and manage performance improvements.
