UnitedHealth Group is seeking an On-Call Hospice Registered Nurse to provide after-hours clinical support, triage patient needs, and coordinate care across remote and field settings. This role involves triage by phone or in-person visits, collaborating with interdisciplinary teams to address urgent patient needs and ensure continuity of care.

Responsibilities include documenting interactions, educating patients and caregivers on symptom management and care plans, and coordinating with physicians

Qualifications

• Current RN license is required.
• Valid driver's license and reliable transportation.
• Current CPR certification.
• 1+ years nursing experience, preferably in hospice/home health/acute care.

Responsabilités

• Provide on-call nursing support, triaging patient needs and determining appropriate interventions.
• Assess patient conditions via phone and/or in-person visits and elevate care as needed.
• Coordinate with physicians, care teams, and caregivers to ensure timely and effective care delivery.
• Deliver patient and caregiver education related to symptom management and care plans.
• Maintain accurate and timely documentation of all patient interactions and clinical decisions.
