Job summary

Optum is seeking a Hospice RN On-Call to provide after-hours clinical support, triaging patient needs and coordinating care to ensure continuity and quality outcomes. This role combines remote and field-based nursing support, assessing patient conditions and collaborating with interdisciplinary teams to address urgent and emergent patient needs.

The salary range for this role is $79,600 to $119,400 annually for full-time employment, with a comprehensive benefits package and 401k contributions.

Qualifications

• Current RN license in the state(s) you are assigned to work.
• Valid driver's license, reliable transportation, and liability insurance.
• Current CPR certification.
• 1+ years of nursing experience, preferably in hospice, home health, or acute care.

Responsibilities

• Provide on-call nursing support, triaging patient needs and determining appropriate interventions.
• Assess patient conditions via phone and/or in-person visits and escalate care as needed.
• Coordinate with physicians, care teams, and caregivers to ensure timely and effective care delivery.
• Deliver patient and caregiver education related to symptom management and care plans.
• Maintain accurate and timely documentation of all patient interactions and clinical decisions.
