Job summary

UnitedHealth Group is seeking an experienced Registered Nurse - Hospice On-Call RN for after-hours coverage. This role combines remote triage with field-based visits to ensure continuity and quality of care for hospice patients.

Responsibilities include on-call triage, patient assessment by phone or visit, care coordination with physicians and care teams, and patient education. Current RN license and CPR are required, with 1+ year of hospice or home health experience.

Qualifications

• Current RN license in the state(s) you are assigned to work.
• Valid driver's license, reliable transportation, and liability insurance.
• Current CPR certification.
• 1+ years of nursing experience, preferably in hospice, home health, or acute care.

Responsibilities

• Provide on-call nursing support, triaging patient needs and determining appropriate interventions
• Assess patient conditions via phone and/or in-person visits and elevate care as needed
• Coordinate with physicians, care teams, and caregivers to ensure timely and effective care delivery
• Deliver patient and caregiver education related to symptom management and care plans
• Maintain accurate and timely documentation of all patient interactions and clinical decisions
