Job summary

UnitedHealth Group is seeking a Hospice RN On-Call to support after-hours clinical needs for patients in the Woburn area. The role combines remote and field-based nursing to triage, assess, and coordinate care to ensure continuity and quality outcomes.

You will work 7 days on and 7 days off, with shifts including weekday evenings and weekend coverage, collaborating with physicians and care teams, and educating patients and caregivers on symptom management and care plans.

Qualifications

• Current RN license valid in the states you work.
• 1+ year nursing experience, preferably hospice/home health/acute care.
• Current CPR certification is required.
• Valid driver’s license, reliable transportation, and liability insurance.

Responsibilities

• Provide on-call nursing support after hours, triaging patient needs and determining appropriate interventions.
• Assess patient conditions via phone and/or in-person visits and elevate care as needed.
• Coordinate with physicians, care teams, and caregivers to ensure timely and effective care delivery.
• Deliver patient and caregiver education related to symptom management and care plans.
• Maintain accurate and timely documentation of all patient interactions and clinical decisions.
