Job summary

The Care Team Hospice seeks an On-Call Registered Nurse (RN) to provide after-hours hospice care to patients and families during the weekdays. Work from home when not visiting, with autonomous clinical decision-making and support from an experienced interdisciplinary team.

Responsibilities include responding to after-hours needs, completing visits, symptom management, and EMR documentation, while coordinating with the care team to ensure continuity of services and compassionate end-of-life care.

Qualifications

• Active and unrestricted RN license in the state of practice required.
• Minimum of one year of nursing experience; hospice/home health/palliative care preferred.
• Strong clinical assessment, symptom management, and critical-thinking skills.
• Ability to work independently, prioritize patient needs, and make confident clinical decisions.
• Excellent communication and compassionate patient-centered approach.

Responsibilities

• Respond to after-hours patient and caregiver needs with timely nursing assessments and interventions.
• Complete scheduled and urgent visits, addressing changes in condition and providing end-of-life care.
• Support patients and families through symptom management, education, crisis situations, and transitions.
• Collaborate with physicians, pharmacies, caregivers, and interdisciplinary team to coordinate care and ensure continuity.
• Complete accurate EMR documentation in a timely manner.
