Job summary

The Care Team Hospice is seeking a Weekend On-Call Registered Nurse to provide after-hours nursing support for hospice patients at home and in facilities. This role blends independence with team collaboration, delivering symptom management, crisis intervention, and education while responding to patient needs remotely when not actively in the field.

Qualified RNs will have at least 1 year of nursing experience, EMR familiarity (HCHB preferred), and strong clinical and communication skills to

Qualifications

• Active RN license in state of practice required.
• Minimum of 1 year nursing experience; hospice/home health, oncology, palliative care or critical care preferred.
• Strong clinical assessment and symptom management skills.
• Ability to work independently and make sound field decisions.
• Experience with EMR; Homecare Homebase (HCHB) preferred.
• Excellent communication and interpersonal skills; supportive teamwork.

Responsibilities

• Respond to after-hours and weekend patient, family, and caregiver needs with timely nursing assessments and interventions.
• Perform on-call visits to assess condition, manage symptoms, and update care plans.
• Coordinate care with physicians, caregivers, pharmacies, and interdisciplinary team members.
• Educate patients, families, and caregivers on medications, symptom management, and end-of-life care.
• Facilitate admissions, deaths, and urgent clinical situations per hospice policies.
• Document patient assessments and interventions in the EMR.
