Job summary

UnitedHealth Group is seeking a Hospice RN On-Call in Garner, NC to provide after-hours clinical support. This role delivers remote and field-based nursing care, triages patient needs, and coordinates with interdisciplinary teams to ensure quality outcomes.

You will assess conditions, educate patients and caregivers, and document all decisions. Salaried position with mileage reimbursement and a comprehensive benefits package, including 401(k) contributions.

Qualifications

• Current RN license, state(s) you are assigned to work.
• 1+ year of nursing experience, preferably in hospice, home health, or acute care
• Current CPR certification
• Valid driver's license, reliable transportation, and liability insurance

Responsibilities

• Provide on-call nursing support, triaging patient needs and determining appropriate interventions
• Assess patient conditions via phone and/or in-person visits and upscale care as needed
• Coordinate with physicians, care teams, and caregivers to ensure timely and effective care delivery
• Deliver patient and caregiver education related to symptom management and care plans
• Maintain accurate and timely documentation of all patient interactions and clinical decisions
