Job summary

UnitedHealth Group is seeking a Hospice RN On-Call to deliver after-hours clinical support in home health settings. You triage patient needs, coordinate care with physicians and care teams, and support patients and families to ensure quality outcomes.

The role combines remote and field-based nursing with timely documentation and education. Required: active RN license, CPR certification, valid driver’s license, and 1+ year of nursing experience in hospice/home health/acute care.

Qualifications

• Current RN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.
• 1+ years of nursing experience, preferably in hospice, home health, or acute care.

Responsibilities

• Provide on-call nursing support, triaging patient needs and determining appropriate interventions.
• Assess patient conditions via phone and/or in-person visits and escalates care as needed.
• Coordinate with physicians, care teams, and caregivers to ensure timely and effective care delivery.
• Deliver patient and caregiver education related to symptom management and care plans.
• Maintain accurate and timely documentation of all patient interactions and clinical decisions.
