Job summary

Affinity Health Management seeks a compassionate Registered Nurse to plan, organize, and direct hospice care in home and community settings. The role emphasizes education, care coordination, and hands-on nursing to meet patient and family needs.

You will assess patients and families, develop and revise care plans, administer treatments, counsel on self-care, and collaborate with an interdisciplinary team. A valid RN license, CPR certification, and driving privileges are required; on-call duties

Qualifications

• Graduate of an accredited school of nursing.
• Licensed Registered Nurse in good standing.
• Experience: 1 year within last 3 years OR a BSN with 2+ years in public health/home care/hospice preferred.
• Supervisory experience not required; may supervise hospice aides when needed.
• Offers contingent upon satisfactory references and background checks.
• Excellent observation, verbal and written communication; math and nursing skills per competency checklist.
• Must be able to lift/move patients and perform required nursing tasks.
• Must have a valid driver's license and insured vehicle.
• CPR certification required (hands-on preferred).

Responsibilities

• Complete initial and ongoing comprehensive assessments of patient and family to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Develop and revise plans of care with patient and family involvement; initiate and adjust care plans as needed.
• Administer medications and treatments as prescribed.
• Counsel patient and family on nursing and related needs; provide self-care instructions.
• Assist patient with activities of daily living and support self-sufficiency.
• Act as Case Manager for assigned caseload.
• Document accurate clinical notes and report changes to physicians.
• Coordinate care with the interdisciplinary team and community resources.
• Participate in on-call duties and ensure equipment and services are arranged.
