Qualifications

• Graduate of National League of Nursing accredited school of nursing.
• Current licensure in State.
• Minimum 1 year of professional nursing experience in last 3 years or BSN with RN license; 2 years in public health/home care/hospice preferred.
• Management experience not required; may supervise hospice aides.

Responsibilities

• Complete initial and ongoing comprehensive patient/family assessments.
• Provide professional nursing care and administer prescribed medications.
• Develop and revise care plans with patient and family involvement.
• Coordinate patient care as case manager when assigned by supervisor.
• Maintain accurate clinical notes and communicate with physicians and care team.
• Participate in on-call duties and ensure equipment and services are available.
• Contribute to quality improvement activities.
