Qualifications

• Graduate of National League of Nursing accredited school of nursing.
• Current licensure in State.
• RN must have 1+ year of recent professional nursing experience or a BSN with current RN licensure; 2+ years in public health/home care/hospice preferred.
• Management experience not required; supervision of hospice aides may be involved.

Responsibilities

• Complete initial and ongoing comprehensive patient and family assessment to determine hospice needs.
• Provide professional nursing care using the nursing process and plan of care.
• Develop and revise care plans with patient/family involvement as needs change.
• Administer medications and treatments as prescribed, and counsel on health care needs.
• Coordinate patient care as case manager with the interdisciplinary team.
• Educate patient and family on self-care, medications, and disease management.
• Attend interdisciplinary meetings and provide safe, compassionate hospice care.
