Qualifications

• Graduate of National League of Nursing accredited school of nursing.
• Current licensure in State.
• RN with 1 year of experience in last 3 years, or a BSN with relevant experience.
• Experience in public health, home care, or hospice nursing preferred.

Responsibilities

• Complete initial and ongoing patient and family assessments to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Develop and revise the plan of care with patient and family involvement.
• Administer prescribed medications and treatments.
• Coordinate patient care as Case Manager when assigned.
• Maintain accurate clinical notes and communicate with physicians.
• Participate in on-call duties and ensure equipment and services are available.
• Supervise hospice aides and delegate tasks as needed.
• Engage in ongoing professional development and quality improvement activities.
• Support emotional needs of patients and families and coordinate interdisciplinary care.
