Job summary

Gentiva is seeking a compassionate Hospice RN / RN Case Manager to serve patients in their homes and allied settings. You will lead comprehensive assessments, deliver skilled nursing care, and coordinate care with an interdisciplinary team to ensure high-quality, patient-centered end-of-life services.

Responsibilities include developing personalized care plans, educating families, and documenting care activities while adhering to regulatory standards and infection control protocols.

Qualifications

• Must be a licensed Registered Nurse.
• Experience in hospice, palliative care, or home health is preferred.

Responsibilities

• Perform comprehensive patient assessments and reassessments including in-person/telephonic RN assessments.
• Deliver skilled nursing interventions and symptom management.
• Develop and update individualized plans of care including visit frequency.
• Coordinate care with interdisciplinary teams.
• Educate patients and caregivers about disease, end-of-life care, medications, nutrition and self-care.
