Job summary

Gentiva Hospice is seeking a Registered Nurse Case Manager to provide skilled nursing care in patients’ homes or equivalent settings. You will guide patients and families through the hospice journey with compassion, coordinate interdisciplinary care, and document all activities to maintain quality and compliance.

The role requires an active NC RN license and at least 1 year of hospice/home health experience. A strong emphasis on patient-centered care and caregiver education is essential.

Qualifications

• Active RN license in North Carolina
• 1 year of hospice, palliative care, or home health nursing preferred
• Ability to assess and modify plans of care with high accuracy

Responsibilities

• Perform comprehensive patient assessments and ongoing re-assessments.
• Deliver skilled nursing interventions and symptom management.
• Develop and update individualized plans of care and visit schedules.
• Coordinate with physicians, social workers, and other IDG members.
• Educate patients and family caregivers on disease processes, end-of-life care, and medications.
• Document care activities accurately and ensure compliance with regulations.
