Job summary

Gentiva is seeking a Hospice RN / RN Case Manager to lead in delivering personalized hospice and palliative care in patients' homes and communities. You will serve as the primary nurse coordinating care with families and the IDG.

Responsibilities include comprehensive assessments, care planning, interdisciplinary collaboration, documentation, and caregiver education, with emphasis on compassionate support during end-of-life.

Qualifications

• RN license in the state of employment
• Minimum 1 year RN experience (hospital, hospice, home health or related)
• Hospice/palliative care experience preferred

Responsibilities

• Perform comprehensive patient assessments and reassessments, including in-person and telephonic visits.
• Deliver skilled nursing interventions and symptom management per plan of care.
• Develop and update individualized plans of care with visit frequency and risk identification.
• Coordinate care across IDG and with physicians and other clinicians.
• Educate patients, families, and caregivers about disease, end-of-life care, medications, nutrition and self-care.
• Document timely clinical records and ensure regulatory compliance.
• Support on-call and patient support activities as applicable.
