Affinis Hospice is seeking a qualified Registered Nurse to join our mission‑driven team. You will deliver professional nursing care to Hospice Services patients, assess needs, and collaborate with physicians and an interdisciplinary group to plan and implement comprehensive care.

You’ll educate families, ensure accurate documentation, and participate in ongoing training to uphold the highest standards of compassionate, patient‑centered care.

Qualifications

• Graduate from an accredited School of Nursing.
• Currently licensed in good standing as an RN in Georgia.
• Six months acute care RN experience minimum.
• Over one year RN experience preferred.
• Home care or hospice experience preferred.
• Current CPR certification.

Responsibilities

• Provide professional nursing care to Hospice Services patients.
• Assess, plan, implement and evaluate the nursing process for caseload.
• Initiate communication with physicians and care teams to coordinate resources.
• Perform initial assessment within 48 hours and comprehensive assessment within 5 days.
• Maintain up‑to‑date patient records and document care activities.
• Develop comprehensive care plans with the Interdisciplinary Group.
• Manage pain control and symptom management in hospice care.
• Educate patients and families about care needs and coping.
• Provide 24/7 coverage and on‑call support as needed.
• Participate in IDG conferences and ongoing staff training.
