Job summary

Affinity Hospice Management LLC is seeking a registered nurse for comprehensive hospice care at patients' homes and communities. The RN plans, organizes, and directs care with emphasis on education and patient-family cooperation, using the nursing process and a collaborative interdisciplinary team.

Responsibilities include assessments, care planning, medication administration, and patient/family counseling.

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse licensed to practice in good standing.
• 1+ year recent nursing experience; public health/home care/hospice preferred.
• CPR certification required.
• Driver with insured vehicle.
• Excellent observation/communication skills.
• Ability to lift and assist patients.

Responsibilities

• Perform initial and ongoing patient/family assessments to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Assess and reevaluate patient/family needs and revise the care plan as needed.
• Initiate and adjust the plan of care as patient status changes.
• Develop care plans based on nursing diagnosis and involve family in planning.
• Administer medications and treatments as prescribed by physician.
• Counsel patient and family in nursing and related needs.
• Provide health care instructions and education.
• Assist patient with activities of daily living and support self-care.
• Serve as Case Manager as assigned by Clinical Supervisor.
• Participate in on-call duties and ensure equipment/services are available.
• Supervise ancillary personnel and delegate responsibilities.
• Pursue ongoing professional development and continuing education.
