Job summary

Affinis Hospice is seeking a skilled Registered Nurse to provide professional nursing care to Hospice Services patients in Georgia. The role requires patient-focused care aligned with physicians' orders and hospice policies, with on-site responsibilities.

Responsibilities include coordinating care with the Interdisciplinary Group, maintaining up-to-date records, guiding Nursing Assistants, and participating in on-call coverage to support patient and family needs throughout the care continuum.

Qualifications

• Graduate from an accredited School of Nursing.
• Licensed RN in Georgia.
• Six months acute care RN experience or as required by state regulations.
• CPR certification.

Responsibilities

• Provides professional nursing care to Hospice Services patients.
• Assesses and plans patient care in collaboration with attending physicians and the Interdisciplinary Group.
• Maintains up-to-date patient records per policy.
• Coordinates care and communicates with Hospice staff and other agencies.
• Supervises the CNA plan of care every 14 days and documents supervision.
• Participates in IDG conference and on-call coverage as needed.
