Job summary

Affinity Hospice Management LLC is looking for an experienced registered nurse to plan, organize and direct hospice care in patients' homes and communities. The RN will perform comprehensive assessments, develop individualized care plans, and collaborate with families to meet needs.

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse with a license in good standing.

Responsibilities

• Complete an initial, comprehensive and ongoing assessment of patient and family to determine hospice needs.
• Provide professional nursing care by utilizing all elements of nursing process.
• Develop a care plan with goals based on nursing diagnosis and plan of care, including patient and family.
• Administer medications and treatments as prescribed by the physician in the plan of care.
• Counsel patient and family in meeting nursing and related needs and provide health instruction.
