Job summary

Affinity Hospice Management LLC seeks a Registered Nurse to plan, organize and direct hospice care in home and community settings. You will perform comprehensive patient assessments, develop individualized plans of care and coordinate services with an interdisciplinary team.

You will provide nursing care, educate patients and families, and participate in on-call duties to ensure high-quality hospice support and symptom control in the home environment.

Qualifications

• Graduate of National League of Nursing accredited school of nursing.
• Current licensure in State.
• RN with minimum 1 year of experience within last 3 years OR BSN with 2 years in public health/home hospice preferred.

Responsibilities

• Perform initial, comprehensive and ongoing assessments of patients and families to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Develop and revise care plans with patient and family involvement.
• Coordinate patient care as case manager when assigned.
• Communicate with physicians and care team to ensure continuity of care.
• Teach patient and family self-care techniques and health counseling.
• Participate in on-call duties and ensure equipment and services are available.
• Maintain accurate clinical notes and records of pain and symptom management.
